The title for this post comes from yesterday's report released by the National Association of Area Agencies on Aging (n4a). It would not be overstating things to say this report is required reading for all of us in aging services. However, if you prefer the abridged version, head straight for the report's Executive Summary, then scroll through the document to read the sidebars. Each sidebar highlights an exemplary program, such as the partnership between Hebrew Senior Life and Elder Services of the Merrimack Valley. My personal favorite is Project Independence out of Marin County, CA, featured on page 20. Only 6% of Project Independence clients are re-admitted to hospitals within 30 days of discharge, compared to more than 20% nationally. It comes as no surprise that Project Independence relies on social workers, who are helping measurably to contain costs.
